I Don't know how I did this, perhaps I was logged in as guest but I lost a form I need to edit.

I did check your web page where you have embedded your form and found that it is associated with a guest account. There are several other forms in your guest account.

I have gone ahead and moved all these forms to your account with username "dtxlondon".
