Hi Martin,
You can disable the or remove the "area code" and "phone number" sub-label field by double clicking it then erasing the text.
You can also inject this custom css code.
#sublabel_area, #sublabel_phone {display:none;}
This guide might be useful for reference:
How to Inject Custom CSS (http://www.jotform.com/help/117-How-to-Inject-Custom-CSS-Codes-to-your-Form)

If you want to remove the hyphen or dashes then you can do it by making sure that input mask is enabled and then you can format it how you want it to work.
Click the Phone field, click on "Settings" gear icon. Then go to "Show Properties"
Under "Properties" make sure that Input Mask is "Enabled". You will see that Mask Value can changed the format. So just remove the hyphen like this - ##########
Hope you find these solutions helpful. Thank you!
