Hi Team,

I need a form which acts like a report. For example with the below fields

TO - (CLIENT EMAIL)

DATE

TIME

WHAT WAS LEARNT IN THE SESSION

HOMEWORK SET?

After each session the tutor had to write a reports and hit send.

My question is can the form be sent to different email address along with us "cc" in ?

Yes. It is possible to create such form using JotForm. Please see the demo form if it is similar that you are looking for.

You can set notifications and auto-responders to the form that can send email alerts with the information entered in the JotForm. Please refer to our guides below for setting up notifications on your JotForm.

You may clone this form to your account and make necessary changes.

By the way, I see that you are having a guest account which are limited to receive only 10 submissions. We recommend you to register a free account with us so that you can upto receive 100 submissions every month.

This is great! Ok so we have around 300+ tutors who would need to complete a form after each session.

Ok a few questions -

1 - How do you recommend they access the site?

2 - I would prefer a link? What options do they have?

3 – Can I login and send this to myself so I can see what the client receives?

4 – Can I add images? Or options to add attachments?

Hello,

Please check my responses below.

1 - How do you recommend they access the site?

This merely depends on you, the form will work the same way if you provide a direct link to your clients or if you embed the form in your website.

2 - I would prefer a link? What options do they have?

If you send them a direct link then they will see the form just like you did when you checked Mike_T's and Kiran's demos.

The embed option is mostly used when you have a website and you want to use your own domain name to share the form with others.

3 – Can I login and send this to myself so I can see what the client receives?

The form is public, so you don't need to login to access it.
Your form will have a public URL assigned and you and your clients will be able to access it just like you did with the demos.

4 – Can I add images? Or options to add attachments?

Yes, you can add Images and Upload Fields (to add attachments). Please refer to this guide: http://www.jotform.com/help/46-Quick-Overview-of-Form-Fields

Further questions –

1 - My worry is if we embed it on the website, will the client or us receive spam?? Bot blasters?

2 – is the form mobile friendly on mobile\cell\ipad\tablet devices?

3- Can I edit the “submit” message for example “Thank you for sending your report”
