How to access a jotform in Tumblr

I have created a form and published it on Tumblr. Now how do I go into Tumblr and see it and use it?

I have guys in the field that will need to use the form I created, complete on their smartphone, and submit.  Then I want to go into JotForm daily and view submissions.  After they install Tumblr on their phone, how do they get to the form?

After embedding the form in Tumblr click on the post:

Copy the url and send it to the preferred recipients:

To view the submissions, please go to Myforms > select form > submissions:

If you set up the email notification you can also receive in your email a copy of the submission.
