Form calculation not working

I"ve created a form called "Thyroid Quiz" and want a simple calculation/sum of the fields above it. I have followed all the steps as I see them, but the calculation is not happening. I have not embedded this form on any site yet.

I've fixed it for you. In the calculation advance properties, uncheck "Wait for input before including the field in calculation." option:
