Email Alerts: Not receiving submitted form filled

I am still not receiving email alerts when the form is filled out. I tested the notification email and received the test, but thats it.

Upon checking your account, the email you are using for the email alerts are not in our bounce list.

Also, I see that in your email alerts setting that you are missing one field.
