    Hi,

To do that, you should edit your jotforms notifier body.

I have cloned your jotform and made the edits here so that you can clone it and check them out or try and see if that is what you were after (the emails will be sent to you with the change).

    This is the jotform: http://form.jotformpro.com/form/43004916603952

    Now the steps to take.

When you open the jotform for editing, go to Email Alerts button in the toolbar and click to edit the email body of notifier/autoresponder that you would like to edit.

Now when you open it just click and edit any part of the email body that you would like

As you can see I had merged the two cells into one and added the header of the data.

You could also make the fields under the header fall in more to the right if you want or just change the style of any section in there, by utilizing the toolbar at the top.

Once you make all the changes, it is best to use the Test Email button and see how it looks.

Do let us know how it goes and if you have any further questions.

    Best Regards,
    Ben
