I'm truly very sorry about the mix up I had. I misread your question that lead to the confusing I had with what you were trying to do when creating a new form like that one.

However, I realize now that I think what you actually were asking about is making a jotform that is very similar to the one in the link you first provided if that might be correct?

Something like this maybe - http://www.jotformpro.com/form/43028163188960

If you want us to change anything or fix it by all means please let us know and we'll gladly help change it in anyway we can to get you squared away and happy with it.

If you're wondering how I did that here's some quick instructions on how to do it:

    (1) Make form with 8 Textboxes.

    (2) Use "shrink" so that your textboxes line up 2 at a time side by side.

    (3) Now use our form designer feature which will help you shift the field spacing closer together. Here's some videos to show you how to use our form designer tool.

If you're happy with the way it is just clone it and it's all yours!

    Hi,

I've finally made a form. But there is a little problem again: a script
    error
I truly hope it is the last problem with my form. I'm sorry but I have
    enough

Sorry to see that you're having an error with it's script now.

Is this happening within your jotform account when previewing it? Or have you embedded it?

What's the script error message saying? And where are you seeing it?

Are you able to provide a screenshot of it if possible?

If you're able to take a screenshot please use our Image Insertion Tool to add it to this thread. This way we can see what's going on and then do what we do best to help resolve it.

Embedded. The problem seems to be when we click on Submit. It's written
(sorry) in French: constante de chae non termin. I join a screenshot.

    Thank you

No need to apologize and we'll do whatever it takes to get to the bottom of the problem for you.

Since you stated it's "embedded" is it possible for you to share a link to the webpage that you have the new jotform embedded on? I would like to inspect it to help you furthermore with it.

From the sound of it since you said it's embedded there is the possibility of a conflict between your website scripts and the form scripts so that could be a probable cause. If you've used the default script method to embed your jotform I would suggest trying to use the iFrame instead to see if that helps at all since it surrounds your jotform in a protective frame.

You mentioned that it's when the problem happens when you click the submit button.

Did you maybe mean that's when the error appears?

Is it not submitting or is there a red text that appears saying there are errors on the form please go back and fix it? Is it something along those lines?
