Upon checking your form, I see that you have several broken conditions on your fields.
And because of that the correct answers are not being counted when they're selected.

I'm referring to these fields.

And these conditions among others.

Also the condition for your total score field is broken.

For this particular field, I suggest that you use the Form Calculation widget instead of conditions, as I think it will be easier for you to just add the values of the fields that you're using in your form to calculate the points you give for each correct answer.

I've deleted all the broken conditions and it's still returning a calculation value of 0%

I've checked your form and it seems that you are now using the Form Calculation widget. 

Are you still having problems with it? The form calculation that you have seems to work fine.

You can unhide the score field so that you may know if the calculation is being made. Also, I noticed that your options are enabled to be "Shuffled".

We are having the same issue with both forms, as well as all of the other forms as well

The issue is not specific only one form
