    Hi ClaraTeo,

FIrst of all, thank you for using JotForm.

You can do that by using JotForm's Conditional Logic. Just navigate to your form, click on "Conditions" under the "Setup & Embed" tab, and choose "Show / Hide Form Field".

    Then construct your conditions.

Your final output should be something like this one below:

You can have the live view of my sample form through http://www.jotform.net/form/12624405212?. Hope you find this useful.

If you have further issues and concerns. Please feel free to contact us again.

I have set the conditions according to the question; however, the option does not appear as accordingly.

    Pls advise?

    Thanks for your help!

    Hi ClaraTeo,

We appreciate your prompt reply. To address your issue, the ampersand sign in the "Beauty & Wellness" and "Home & Living" items are causing the problem. I recommend you replace "&" with "and" instead and recreate your conditions. That should help. You can check the changes I've made to the form I've cloned from you through http://www.jotform.net/form/12641348255?, if you want, you can clone it, too.

If you have further issues and concerns, please feel free to contact us again.
