    How to have customize output of JotForm submissions

    Presently my jotform submissions, when downloaded, look like this:



    Is there a way to modify the output to look more like this?

    Can you please tell us how did you come up with this custom submission output?


    I checked the PDF submission output and it is not like that.

    Assuming you wanted it to be in the email submission, you can then customize the Notifier email template to have this output




    You can use the tools available in the Email Editor to customize the email template

    For now the online chat is not available.

    I created a clone in your account. It's called Clone of ADULT ONLINE INTAKE. Its Notifier 2 is similar to your request.

    Although it is possible to customize the email notifications they will stop updating if you add/remove fields from the form. This way if you edit your form it is necessary to either manually add/remove the field or to create a new email notification.
