Why are my forms Disabled?

    As i havent reached the 100/month limit, why are my forms offline?

     

    Afert clicking enviar, it shows form disabled

    I checked your account and you had a form with a password field which is not allowed according to our terms:

    I deleted the password form and activated your account.
