I looked into MachForm too, but it says it doesn't work with squarespace website templates and I am looking for a form for my photography clients to fill out and pay for their sessions when we book a session. I will be watching for a response as I am hopeful this would work with my business!

Thanks for your interest in JotForm. I believe our forms can be embedded to Squarespace, this tutorial or guide that we have will help you in embedding our forms in that website: http://www.jotform.com/help/37-Adding-Form-to-Squarespace
I hope this helps. Let us know if this works for you. Also, I would recommend that you register for an account so that you can explore more our Form editor and so that you can fully take advantage of the forums.

I am not sure what I am not getting about this, but I am totally in the dark. Is there anyone that can walk me through this?? I have no website building skills so I don't even understand the iForm thing.

I see that you have already created a form. You want this form to be added to your SquareSpace website. Is that correct?
You should take a look at the following guide where you can find step by step guide how you can embed your form to SquareSpace website: http://www.jotform.com/help/37-Adding-Form-to-Squarespace
<iframe id="JotFormIFrame" allowtransparency="true" src="//form.jotform.us/form/43220418497152" frameborder="0" style="width:100%; height:3977px; border:none;" scrolling="no"></iframe>
Hope this helps.
