Can your forms be coded to use calculated fields like subtotal and total? Can they generate a variable number of fields based on quantity input, for example a use enters 5 for number of members, and the form then prompts for input of five names and birthdays? Can I set up variable pricing based on input? For example, a user inputs birthday. If that member will be under age 3 by memorial day, their cost is $0, else their cost is $50.
These are the types of controls I have been looking for in past versions of form builders, but have not yet been able to move our membership forms off paper. I look forward to learning about your capabilities.
Good luck with your venture,

It is possible to use JotForms for calculating subtotal and totals. Our Form Calculation widget enables to perform calculations using the values entered on the form.
Hope this information helps.
I've moved your other questions to new threads and shall be addressed separately.
Variable number of fields based on quantity input....
