I spent 8 hours today working on a very nice web form for my website, all to find that a few minutes after finishing the form, my account, er... my form has been disabled. I am certain it is due to JotForm security procedures as it relates to sensitive/login information etc. I believe I typed 'username' or something similar in one of the forms, however I immediately deleted it when I realized the error. I am finalizing some work on a secure portion of my website and had several web form platforms open at once.
I have no interest in obtaining any sensitive information or anything of the like. Please review the forms on my account to confirm I am not requesting sensitive data and reactivate my forms asap as I am a small business owner and am in dire need of them.

Hello Eric,
Upon checking your account I found that it's suspended by our anti-phishing system. We have automated anti-phishing system which scans all forms and mark them with probability of being used for phishing activities.
Upon taking a closer look at your form, I can confirm that it's not being used for any phishing activities. It seems you have already deleted the password field. I have activated your account now.
