File uploader not working

Hello, 

I was having trouble with my form. My PDF weren't visible when I actually tested it from my site, but were visible in preview mode. I attempted to reload, by using file uploader, however, it would not allow me to choose any files. 

I ended up saving as JPEG as the PDF widget didn't produce the desired results.

When I went to your website above ( http://woodlandsrugby.org/ ) all I saw was hidden iframe with a link to jotform answers.

I took a look at the contacts page as well and was not able to see any jotforms there neither.

Can you please give us a direct link to the page where the jotform in question is and the steps to recreate the issue.
