How can I create a form for teachers to keep track of time?

    I work with school employees and we want to be able to collect hard data on how many minutes are lost each day to meetings, trainings, and any other things that take away from direct instruction.  I'd like to be able to provide them wih a form to track 25 school days and have the form tally the minutes lost each week.

    The time will be entered manually?
    I mean, every teacher will have to manually enter how many time he spent on meetings, trainings, etc?

    If this is the case, I would probably go with creating a dropdown that contains the names of each teacher and then have a checkbox for them to select if the entered time correspond to a meeting, training etc. and have a slider so they can enter how many minutes they've spend on the selected activity selector and a Date picker to track the date of the event.
