    It vital that I regain access to my forms and account. This misunderstanding resulted from me sending too many test emails to myself.

    Hi Daryl,

    The reason why it was disabled is because one of your forms is asking for some sensitive information like account password. This violates the terms of use of JotForm. Note that JotForm does not have account database features that's why a password field is not usually needed. You may consider using Profile Manager for such applications instead. Please visit http://interlogy.com/products/pmb/ for more information about this product (from the same guys that gave you JotForm).
