Form not working on mobile

    Hello, my form on this page shows on my desktop but on my mobile phone (android) it has the message "Form not available". Can you tell me how to fix this please? http://www.kryzma.com/4331/contact

    Please add our Mobile Responsive widget to you form. Here is how:

    Go to Widgets Menu > Type "Mobile" and you'll find it.
