How I can reclaim an erased form accidentally

I am not sure if I have understood your question correctly. Do you mean to say that your form fields are deleted or you have deleted the form itself?

If the fields of your forms are deleted, you can take advantage of our "Revision History" feature to revert back to an earlier working version of your form. This will restore the form field.

If the form itself is deleted: Please be noted that when a form is deleted, it is simply moved to "Trash " folder and not deleted permanently. Forms can be restored from the trash folder. Please check the screenshot below:

Please be noted that you should NOT click on the "Purge" button. It will delete your form permanently.

I have anyway restored the deleted form of your account.
