How to add modified source code as a lightbox on my website

At the same time, I'm using form lightbox on my website. Thus, what should I do in order to use my form with adding days script directly in lightbox on my website? I tried to upload new page and change "base", but it doesn't work. Please, show me an example of lightbox code.

I do not think that we can combine a custom script with our LightBox embedding option.

You might consider using the form source code and your custom script on some separate website page. Then load that separate page via iFrame using any third party LightBox library/code.
