ERROR: This form is currently unavailable!

When I submit my form I now get the error

This form is currently unavailable!

 

Earlier, I did remove the integration and recreate it with a Google Spreadsheet.  That seemed to work fine and when I created it with a new Google Spreadsheet it populated the sheet with all the existing submissions.  But now when we try to submit a form we get this error.

What is causing this error (maybe it is not related to the Google Spreadsheet)?  How do I fix it?
