Where do I submit my Form for the competiiton?

I am designing a form for the JotForm Form Design competition, but I don't know where I submit my final result.  There doesn't seem to be a submission area on the page or in my account.

You should sign up on our awards page and you will be sent a link in your email with all required details :)

I would suggest going through it to see the rules and judges as well.
