I would like to create a form that will allow the coaches of a basketball league to select a time for team photos from a list of available times.

I want to create a form that will allow coaches to see a list of available team photo times, select one, add the team name, click submit, then view the automatically updated form. 
Is this possible?

The form you want to create seems possible but I'm not exactly sure about what you mean on view automatically the updated form after submission. If you could add more details about it, we'd be glad to review it here.

Anyway, about the photo selection and adding names can be done in the form builder.

1. For photo selection, you can use any of these widgets.

2. Use conditions to show a textbox for adding the team name after selecting a photo.
