Hi,
If the form is embedded, please let us know the URL of the page so we can check further. Viewing the form using its direct URL seems fine:

Firefox:
When you click Preview button, the form detects the window and when you have a small window, the form will become responsive. The form designer is designed to automatically do the responsive design.

It's the form responsive design which is activated after you save your form into the form designer. Please re-embedding your form using our iFrame method: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code. Check if this makes a difference. Please update us here.
