How do I insert an embedded document in my current jotform that is required to be viewed? Ex. Customer Intake form having a release req. to be viewed

    I have 2 jotforms currently.  One is a customer intake form.  The other is a release of liability form.  I would like to have the release form embedded in the customer intake form and has rule built in to it requiring the person to view the release before it will allow them to hit the submit button.

    I understand that you want to have a rule to require the person the release agreement before submit the submit button. I see that the agreement release form fields are already placed inside the other JotForm. At the moment, I am thinking of having a checkbox with conditions to hide the submit button.     Almost. 1. I'm looking to have the release portion hidden behind a link and the customer clicks the link to open the release and then click the checkbox. 2. But they would not be able to click the submit button unless they have clicked the checkbox.

    Using form collapse can be the viable solution using built-in features of JotForm in this case. The agreement and the check box is enclosed in a section which will be hidden inside the section. Clicking on that, the agreement shows up on the JotForm and the user needs to click the checkbox and then proceed with submitting the JotForm.

    Since the checkbox is made required field, without checking this field user cannot submit the JotForm. If he tried to click on submit, it shows an error as below :

    Well, I added it and it was great.  But then I needed to have the number of dogs dropdown to default to 1 rather than blank and after i did that, it got screwy and started added all kinds of fields that shouldn't be there.  But I can only see those fields in the preview, I can't see them on the editable form itself to be able to take them out.  Can you take a look and see?

    Hi,

    Upon checking your form, I see that there are some missing fields and errors in your condition, this might be causing the problem.

    If you scroll down, there are also a couple of missing/deleted fields that might be causing the problem.

    Please do check and fix the conditions you have and try again the form.

    I see that the number of dogs is defaulted to 1. It seems normal to view the JotForm at my end. Could you send us a screenshot of the issue that you are looking at? This would help us in investigating it.

    Also, as Charlie mentioned I request you to take a look at the conditions set on the JotForm as well to see if that causes the issue.

    I also took a look at your form and came up with this version: http://form.jotformpro.com/form/43155113591954 with fixed conditions to achieve what I think you are looking for.

    no, that just made it worse.  sorry.  i tried attaching the screen shots and it didn't work.  i followed those instructions exactly.  don't you have a screen sharing option or something so i can show you.
