I have a pretty complicated little form I created. How much would it cost to have an expert run through it and see why it keeps locking up on tablets

    Also would like to have you guys double check my logic statements.  Can I purchase a one time check up like that?

    Hi,

    I see that it's indeed a very large form, but I'm not pretty sure what do you mean when you say that it "keeps locking up on tablets"? You mean that it doesn't work on mobile devices? like tablets? If this is correct can you tell us what happens when you try to fill the form using one?

    I did a test submission and I was able to complete it successfully and got redirected to this page

    It may be the tablet I was using, on the first page around the middle of
    the form it just wouldn't move to the next question.. But I have an older
    I pad and it may have just been running very slowly since the script is so
    long..
    Thanks for running that test. I want to run through all of the choices,
    but I was hoping you would take a quick look at my logic statements first
    to make sure they make sense and should send an individual to the correct
    portions of the form so we collect all necessary data.

    if I create a thank you page will it
    redirect the patient to my site

    can I use the pre-made HIPPA form as
    an intermediate thank you page?
