I see that it's indeed a very large form, but I'm not pretty sure what do you mean when you say that it "keeps locking up on tablets"? You mean that it doesn't work on mobile devices? like tablets? If this is correct can you tell us what happens when you try to fill the form using one?
I did a test submission and I was able to complete it successfully and got redirected to this page

but I was hoping you would take a quick look at my logic statements first
to make sure they make sense and should send an individual to the correct
portions of the form so we collect all necessary data.
Just two more quick things, firstif I create a thank you page will it, and
redirect the patient to my sitecan I use the pre-made HIPPA form as
an intermediate thank you page?

Per your request I went ahead and check the conditions in your form and couldn't find anything wrong with them.
