How can I replace our Jotform that has been deleted

I have logged in to our "ceresville" jotform account.  It says there are no forms here.

As I mentioned, this form set up as our payment page was orginally designed by Lynton Web 

in Texas. 

How can I get this form back up?

Are you referring to this form?

If this is correct, you should be able to find it in the Trash folder and you can restore it using the Restore button.

However, if you want to avoid issues with this form you need to change it since you're requesting credit card information without using any of our payment tools and this is against our terms of use.

You need to remove the fields that asks for credit card information and use a payment tool instead.
