I want to create a simple customer information update for my business

Hi,

I'm trying to get my customer's contact information updated on a form that i can distribute my hand, mail and email.

I would like to use one of your templates and have a place in the bottom that explains that we also now take visa and mastercard, discover and amex

I am not quite sure what you mean. Could you please give us more details as well as the link to the jotform that has an issue / you want to change?
