Why Is A Form I created Not Showing Up in "My Forms"?

I created a Baptism Request and have posted it on our church's website, but the form does not show up in "My Forms" along with the other forms I created.

How do I get the form into the "My Forms" folder for future use?  I actually do not know where the form is currently located!

Upon checking your form #43098352452153, I found that it is associated with a guest account. This type of accounts are created if you do not sign up / sign in to your account and start building forms.

There are 2 other forms in your guest account. I have moved all the 3 forms to your account "theroadumc". You should not be able to see these forms in your account.
