    Text Boxes Too Wide When Form Opens

    When I edit my Patient Update form text boxes are perfect, but when I open in a new tab or open to use it, the text boxes are too wide for display.  Here is example of Edit Mode:


    This is example of form open for user.  Display is not proper.  Please help]

    Injecting the following CSS should fix the problem:

    .form-textarea {
    max-width: 490px;

    (You can increase or decrease the 490 according to your liking.)
