Keep seeing the forms on my website as disabled

All the forms on my website are all disabled. Please help

I was able to see the problem

Your account and forms were auto suspended by the phishing filter due to a suspected phishing form http://www.myjotform.com/form/43170797721560 that capture Email account password

We do not allow such kind of form to be used.
