How can i make the forms on my site to stop showing form not found

Hi,

I took a look at your account and I see that your account got disabled by our system.

The reason why this happened is because you are asking for email and password, which is not allowed on jotforms as per our terms of use.

You will need to remove the jotform in question or remove the password field and we could then enable your account.
