    Which code do I need to use to use my JotForm in Dreamweaver

    My previous problems were probably because I had reached my free limit and hadn't noticed. Before noticing, I was doing everything I could to make it work, obviously screwing the form up totally.

    I paid the premium yearly, and imported a Dreamweaver form that I re-made.  When I preview it, it doesn't look at all like the form I created that is on the web.

    What can I do outside of totally remaking the form again in JotForm to get it working?

    Are you referring to this form: http://www.jotform.us/form/42735554355156?
    If this is correct, you need to embed the form in your website to ensure that it looks the same way as it does using the direct URL.

    Since you're using Dreamweaver, you need to check the embed code of your form and paste it in the HTML page. I suggest that you take a look at the following guides: http://www.jotform.com/help/67-Which-Form-Embed-Code-Should-I-Use and http://www.jotform.com/help/34-Adding-a-Form-to-a-Web-Page
