Problem with adding Pay by check fields to form?

    I am trying to create a form which has the choice of paying by check or paypal. I think I got the paypal part set up correctly with 2 payment level choices.
    The pay by check option, when clicked on the radio button pulls up the text box that says Pay by Check- but the box next to it where I want to enter the mailing address and instructions for paying by check keeps getting cleared- I can't seem to save it.

    I'm not sure if my understanding is right.

    The problem you are having is that you are unable to save the textboxes or fields, like "Address" and some Text, that you would like to show up when the "Pay by Check" radio button is selected?

    If this is the case, I've cloned your form for testing purposes and added the fields there.

    1. Add the field "Address" for "Check Address" and Text for "Instructions"

    2. In your conditions, if you would like to show multiple fields at once, you can click the "Show Multiple" and choose the fields that you would like to display. Note: To choose multiple field, hold "CTRL" button and left click.

    If you are having problems on saving the changes that you have, I would suggest to clear your browser's cache and log out and log back in to your account.
