JS embed code does not work

    Hi, I am running into another problem!

    The embed code does not work:

    <script type="text/javascript" src="http://form.jotform.us/jsform/43095390340148"></script>

    This is the code for the Baby Dedication Form.  I tested it by pasting it into an e-mail and sent it to myself.  When I clicked on the link, I get a message that page is not available!  I re-saved the form and the code form # did not change.  So what is causing the failed action?

    Hi,

    I tested it by pasting it into an e-mail and sent it to myself.  

    It will not work that way. The embed code can be used only within an html tags.

    Your email client/reader will most likely just filter/erase/clear the script code if you embed it on an email message.

    Now, if you want to include the form URL in your email message, you can do like this instead.

    I also provided the embed code to our website administrator and it did not work either!  

    Are you able to try the link from a webpage?

    I still need assistance with this problem!

    Thank you once again. 

    I need the embed code to work on the church website, not in an e-mail. 

    The church website is:  www.theroadumc.org.  The correct link is pasted below for the "Baby Dedication Form".  Please note that this was previously posted on the website, but I inadvertently deleted the jotform.  You helped me to retrieve it from the trash can.  Now we are trying to upload it again and now we are experiencing problems with the embed code.

    [NOTE: When you visit the website you will find another Baby Dedication form.  I created a 2nd form when I lost the first form.  However, the information collected should be directed to an email address at the church, the form currently posted online is not directed to that mailbox.  The form at the link below was the form directed to the church mailbox.  For this reason I wish to retain the original form for consistency.  Hope that you understand my dilemma.]

    This is the message received from JotForm when I click on the link:

    404 Not Found

    Shereda,

    From what I can see, you have embedded the code correctly but there appears to be an AdCash script which is interfering with the form. Please replace the jotform's script with its iFrame equivalent and let us know if that helps.

    Abajan,

    Thank you.  I have copied and pasted the iFrame code and the form appears to work.  However, once it was on the website and I tried to complete a sample form, I ran across a problem with the "prefix" for "Mother".  So because I could not determine how to get this hiccup to work, I deleted the requirement for "prefix" throughout the entire form.

    I have saved, copied, pasted and submitted the new iFrame code to the website administrator for him to replace.  In the Q&As I learned that every update requires reposting the iFrame code because there are changes to the formatting of the form, or something to this effect.  Tomorrow I will try once again to do complete a test request from the website to ensure that form is working.

    For now, you can close out this ticket!
