How to remove country and state/province from the address control?

in the address bar I do not need the country selector or state/province selector - cannot seem to see how to delete these fields

To go about removing those fields:

1. Select the address control on your form
2. Click Fields button in the toolbar
3. Uncheck the fields that you want removed
4. Click OK then save your form
