Disabled account

    On the 10th November we had our account disabled (I received an email). I just paid for new subscription but I can't seem to log in. Can you help me out please?

    Hi Jo,

    We have reactivated your account and added back the Premium Plan. You've made the payment, but you forgot to include your Jotform Username so it says, "undefined" to our records. You should be all set now.

    Your username is: BirthmarkSupportGroup. If you forgot your password, you can simply reset it using your current email address.
