My form should display a blue arrow in the header image but it isn't.

    Please allow me some time to create a background image for your form that includes the blue arrow as I think it will be easier to work with an image instead of creating this effect using only CSS.

    I have created the background image for your form.

    This is a demo form in which you can see a cloned version of your form: http://form.jotformpro.com/form/43205997512963 with the picture in it.

    In order to add the background image to your form, you need to open the form designer and upload the image to use it as the background of the header text.
