Spoiler with checked input!

I have create a spoiler with image and checked input

but when i select the input and submit in the data collection dont appear the selection!

what is the correct html syntax for collect the data?

I understand that you want to collect data from the select box inside the spoiler widget. Unfortunately, this is not possible since the content inside the widget is text/content and it is not considered to read the input fields inside the widget.

Alternatively, you may consider using our Form Collapse from Survey tools that helps you hiding the content and placing the input fields individually that are allowed to collect data inside form collapse.
