The form is a clone of another form I created, only changing the e-mail and the url of the thank you page.
You need to re-embed again the form using its source code embed on the website. This should fix the issue.
Follow this user guide:
Make sure to clean also all the previous embedded code before re-embedding to avoid any possible conflict.
I am not sure yet the reason why you need to use the source code embed, but if possible you can also just use the default embed code and the form should work fine also.
I mad a test submission using the form URL http://www.jotformpro.com/form/43215864714961 and it works just fine
