    Hi Carol,

    Are you saying that you wanted to create a jotform similar to in the website?

     

    in my assessment, I believe this can be easily done using the Form Builder.

    I suggest you create an account first. Use this link http://www.jotform.com/signup 

    and start creating your form.

    You can check the JotForm Template Gallery for similar form concept and you can instantly use a template.

    Hi there,

    I have a few sites that use the same exact form as above... So I can use the same form on different sites?  All the responses come back to me. So I can use one email address (mine).  How do I distinguish which site it's coming from though? 

    Would the $9.95/month plan be sufficient for me and what I'm looking to do?  Not quite sure what "3 sub-user accounts" mean.  From all the sites with the form above and a contact form, I don't think we receive 1,000 monthly submissions but definitely more than 100.

    I need to move all the forms over seamlessly before the end of the year.

    One more question... Can Google Maps be integrated into the Contact Us form?

    Hi Carol,

    Let me answer your questions here, but if you have an additional, please create a new thread for those.

    -Yes, correct.

     

     All the responses come back to me. So I can use one email address (mine).

    -Yes, you can. Multiple recipients (http://www.jotform.com/help/39-Send-Notifications-to-Multiple-Recipients) in a form is also possible. 

     

    How do I distinguish which site it's coming from though? 

    -You can use our Get Referrer widget for this. Here's the widget: http://widgets.jotform.com/widget/get_referrer. This widget captures the link of the page where the form is submitted.

    -If you think you'll surpass 100 submissions in a month, but not 1000, then Premium (9.95) would be sufficient.

     

    Not quite sure what "3 sub-user accounts" mean.

    -Sub-Account means, the account that can access your existing forms and can create forms on your behalf. Please check this guide: http://www.jotform.com/help/232-How-to-Share-Forms-with-a-Sub-Account-User.
