    Hi JotForm team, 

    I was testing my form and it kept showing " Sorry! Only one entry is allowed. Multiple submissions are disabled for this form." Could you let me know how to solve the problem?

    Another thing is, I have received a submission without upload results/attachments, but the upload fields are required. I tested before and the form could not be submitted without filling out the upload fields. Could it be that the user had uploaded files and submitted the form but I could not see the results, or the user somehow did not upload anything but successfully submit the form? Could you let me know how to solve the problem?

    I've cloned your JotForm to check the issue with allowing only one entry and found that the option Loose Check is enabled under preferences.

    This option restrict the user to submit multiple entries using cookies. Please disable this option so that you'll be allowed to submit multiple submissions.
