Add additional field on a form while filling it out

I have this form that includes 2 fileds.

There are times when I need to submit multiple items on the same form with the same 2 fields.  Is tehre a way that I can keep adding items so they come through on the same notification email?

I am not entirely sure that I understand, but if you would like to have several different - same - fields you could duplicate the ones that you have to match the number of fields that you might need and then as you fill out both the new 2 sets are shown.

If you hide the empty fields from the email, you would only get the fields in emails that got filled out (1 pair, 2 pairs, etc).
