Error messages are displayed when people try to use my form

Hi.

I have had the form on my site since Nov. 1.

Only 1 submission has come through.

I am getting emails from people attempting to use the form stating that they are getting error messages.

Can you please check to see if there is something I did wrong or find the source of the error. Why is it not working??

I did a test submission on your website and was unable to reproduce the issue.

Could you please give us more details about the error messages that your submitters are receiving?
