Visitors may write a review of one of my products on my ReviewCD form. Additionally visitors will have the option to enter their url under their email link.
On the page with the reviews the contributor will have his name beside his remarks with the option to link to his or her email address or url
The form also requests permission to do this
If I understand correctly, you want to allow your users to share their email address or URL in your form. Is that correct?
If so, how about adding a radio button which they can select if they want to share this information with you? Like in this demo form: http://form.jotformpro.com/form/43226219200947 please take a look at it and let us know if this works for you.

That's great . I revised it a little with the option the add a url but must enter email
