I am looking at building a form/page for customers to be able to personalise a their own garment/clothing.

Is this possible to do with the JotForm system?
Currently, I am able to create a form which changes other pieces of information in the same form but not sure if it's capable to change images based on customer choices.
Alternatively if anyone knows what this system is called (If it even has it's own name) or if there's software out there for this, please let me know.
Any help with this would be most appreciated.

You can create this form to Jotform, not really like the given example but should work the same. For example, this form template: http://www.jotform.com/form-templates/purchase-order-point-of-sale.
The form is using our Free Text HTML field for the images and the images are shown based on the choices in the Dropdown field. This method is using conditional logic (show/hide condition) feature.
You can also use any of the following widgets:
Some of these widgets allows you set image size which is a good option for your form requirements.
For the other options such as the Quantity, Colors and Sizing, you can use our Dropdown field.
