Not receiving the pdf of the Approval form

Our Leave/Travel Request form - We have an approval form that we meaning the 2 HR's would receive after the approval. It was a pdf and included all the process. I am attaching one of the previous ones that we received. We like to have this as we can save this to their files for future records.

Ok, so I would just add Approval Report. For some reason it did this automatically before, not sure why it was changed. I will add and hopefully that fixes the problem.
