How can I insert an image either on left or right side of Form?

Hello there,

I am making an e commerce website where it requires filling up the form while looking at the image. Jotform is a great tool to make a form and so far it has helped me a lot.

Now the question is, i want to insert an image either on left or right hand side of form, as buyers can read the image and fill up the form accordingly, since image has numbers which is co-related with form. I have attached file for your reference. 

Any Template / help plz?

I can't find a similar form, but I'm sure there's one to our Form Template Gallery: http://www.jotform.com/form-templates.

We can help you to align the image, just please create the form and make sure you have all your preferred fields to it. Update this thread for the form URL as well as the image URL so we can make the change.

I have already created blank form (Not using template) and i was wondering if you can insert image on my existing form.

Name of the form is "I want my Custom Measurement + $15".

This image looks good on my form. Thank you very much for your help. I will appreciate if you will help me for my other forms too. (User name : fashionsbyindia) 

1) I want to insert following image in my other 2 forms as desribed under. 

a) Fancy Designer Blouse (with cups) + $15

b) Fancy Designer Blouse + $12

2) Cotton Petticoat + $5
